An order, written in a hospital chart after careful and considered discussions, to not attempt to resuscitate a patient if breathing and/or the heartbeat should stop. This is usually considered if the patient, after all reasonable therapeutic efforts have been made and have failed, is suffering greatly and rapidly failing from a far-advanced malignancy. Also "Do Not Rescuscitate" (DNR).
